The Challenge

Clinicians faced a tedious, error-prone process of manually setting patient goals. This resulted in unreliable data, incomplete information, and lengthy processing times.

There was no structured way for clinicians to define goals with actionable steps, or for patients to suggest goals and report challenges back to their care team. The system needed to work seamlessly across both web and mobile.

Understanding the Problem

Our vision was an intuitive, mobile-friendly interface empowering clinicians to define clear goals, map out actionable steps, and prioritize with flags. Inspiration struck during competitor analysis — form builders and accordions offered intriguing potential for streamlining complex data entry.

This innovative approach promised to streamline workflows, improve data accuracy, and empower both clinicians and patients in their journey towards better health.

The Solution

This project aimed to bridge the gap between clinicians and patients through a collaborative goal-setting system. Clinicians can easily define patient goals with actionable steps. Patients can proactively suggest goals and transparently report challenges encountered.

The key challenge during iteration was prioritizing information within the limited viewing window. While maximizing above-the-fold content wasn't feasible, I designed a layout that prioritizes essential actions and readily accessible details.

Through iterative design, I arrived at an intuitive accordion-style form that empowers clinicians to create unlimited goals and nest actionable steps — breaking down goals into manageable steps that foster patient engagement.

This space-saving solution translates seamlessly to the mobile app, ensuring a smooth experience for patients on the go. It streamlines communication, empowers collaboration, and simplifies goal management.

During the final design phase, I crafted comprehensive documentation for a unified user experience across the mobile app, engineering, project management, and stakeholders. Consistent design patterns created a clear and intuitive flow for patients submitting new goals, adding notes, and sharing feedback.

The Outcome

The Goals & Action Plan feature empowered personalized healthcare like never before. Clinicians can create tailored programs specific to each patient's needs. Patients can conveniently record and report their progress directly through the app.
